To about 25 cm, with a blue-grey upper body ruled with fine yellow wavy lines, a yellow rear body and tail, and two dark bars across the head — one through the eye, one behind it. The head bars separate it from the other barred rabbitfishes here.
It grazes algae on reef flats and shallow coral in pairs, and the pair alternates feeding and watching in the manner of the rest of the family. It rests in the reef at night in a blotched brown pattern. The fin spines are venomous. Recorded from about 1 to 30 m.
